By submitting this form, I confirm that I have read and acknowledged the statements below:

1. I acknowledge that I have my consent to share my client's personal data (name and email address) with EEK & SENSE for the purpose of administering the GLWS. EEK & SENSE take no responsibility for the accuracy of the personal data that you have provided for this purpose. Please check this carefully before submitting this form.

2. I acknowledge that I have met the minimum upload requirements for my bundle type. If this minimum requirement has not been met, an additional fee of AUD$55+GST (irrespective of country of purchase) will be invoiced to me.

3. I acknowledge that the number of GLWS surveys will be deducted from my purchased bundles regardless of respondent completion status. You will receive an email confirmation when the GLWS survey links have been administered.

FOR Business Development Requests only: I acknowledge that should my request for a Business Development GLWS be approved, and that I will incur an administration fee of AUD$55+GST (irrespective of country of purchase).

IMPORTANT

One unit of survey and report will be deducted from the remaining quantity available from purchased bundled when a survey link is created. This means that irrespective of whether the survey link is subsequently opened or completed, one unit of GLWS Survey and report will be considered to have been 'consumed'.
